Ø  Nomination Form signed by the candidate and nominator (sections: A, B & D)

Ø  Contact information of two References (section: C)

Ø  Resume of the candidate

Ø  A completed ‘Humanity Service Description Form’

Ø  A digital or print photograph of the candidate (> 1MB)

Ø  An ID/travel document copy of the candidate

*A nominator may nominate multiple candidates for the award
